In a heart-breaking error at a Sydney hospital, a newborn has died and another is in critical condition with serious brain injuries after nitrous oxide was incorrectly administered shortly after birth. An installation error by gas company BOC Limited was to blame for dispensing “laughing gas” instead of oxygen to the infants—however, a formal investigation has been called into whether or not hospital staff followed proper protocol that may have identified the problem sooner. The gas line was installed last year, in 2015.
